Submit/update program SLOs

The purpose of this form is to submit SLO OUTCOME STATEMENTS for new programs prior to Curriculum Committee tech review, or to update outcome statements for existing programs. For new programs, complete and submit the form to receive an emailed receipt, which you should print an attach to the New Program Proposal package that you submit to the Instruction Office.

You will need words to complete UP TO EIGHT outcome statements that begin "students completing this program will be able to...". You will also need for each outcome a list of the required and optional courses whose content (as expressed in the official course outline of record) support that outcome.

To make corrections of any kind, simply resubmit this form.
