In an effort to make SLO assessment easy, useful, and accessible, this form is provided for submitting assessment results.
This does not require being on campus or a "VPN connection" or a specific browser -- just an internet connection.

What's this for? This form produces an email that goes the college's Instructional SLO Facilitator for 2012/2013, Robert Burns, who will update the WebSLOs database on your behalf.
Use it to do any of these things:
submit course assessment results (1 assessment topic at a time)
reword, remove, or add an assessment topic
enter the assessment method to be used for assessing an topic
update an action plan for a previously-entered topic
change the contact person for a course's SLO assessment
What do I need? This process is easy if you have the right documentation gathered before you start.
If you want to ENTER THE RESULTS of an assessment, you need to know
(1) which topic was assessed by its sequence number (like "topic #2"),
(2) the assessment method used and the criteria for determining achievement,
(3) the data summary with how many students were included in the assessment and how many achieved, and
(4) the semester(s) in which the assessment took place. [for example, Fall 2012]
Be sure to describe the cohort.
For courses with more than one assessed section for the reporting period (semester or academic year), submit JUST ONE FORM for the course.
Combine the assessment results, or list them separately per section as your department prefers.
If sections include on-line and face-to-face, PHC and SRC, day and evening, or if they don't, it's best to say so.
You don't have to have a plan of action based on your analysis of the results, because you can enter that later.
Just say something like TBD for the action plan at this time.
Note that saying TBD does not constitute a complete assessment, and assessments are not credited until they are actually complete.
It's just that it's convenient for departments to have some place to record their assessment results before they are able to consider them and form action plans.
If you want to ADD or REWORD an assessment topic, you just need the words to complete the statement "students completing this course will be able to...",
and if you have it, a description of the assessment method to be used and the criteria for determining achievement.
You should base assessment topics on the course objectives/SLOs in the course outline or record.
If you want to REMOVE an assessment topic, you need to know its sequence number.
