Online Services
You will need your InSite portal login to access the forms below. If you need help logging in, please contact the Help Desk at 925-229-6888.
Submit this if you have transcripts, coursework, or AP scores that would clear you from the placement requirements. This will not clear prerequisites.
Use this form if you need to update your Primary Major or Home College location. You are also able to sort for Financial Aid Eligible Majors only.
Use this form if you need to make an edit to the information we have on file.
To give students who are close to graduating an advantage in registering for classes needed in their final semester; apply for an early registration date by submitting this Early Graduation Application in the semester before you plan to graduate.
Use this form if you need to request IGETC or CSUGE verification to the school you are transferring. We only send ONE request so only use this to submit it to the school you're transferring to. These are typically sent when you have received and accepted an offer of admission.
Submit your petition for a degree and/or certificate online through InSite. You can also track the status of your petition!
If you would like to petition to change the grading option for a course to P/NP please submit this form online. Please check with a counselor BEFORE submitting this form. This form can take 48-72 hours to process once received.
If you have coursework completed at another institution that satisfies the prerequisite for a course at DVC, please submit this form online and upload a copy of your transcripts.
This form can be submitted if you have lost priority registration due to being placed on academic probation/dismissal or if you have over 100 degree applicable units.
Please use this form if you are being charged non-resident tuition and believe you qualify for resident tuition.
This form is intended for use if you need to submit sensitive or confidential information to the Admissions and Records Office. If you have questions about documents to be submitted, please email admissions@dvc.edu.
